Moving Home Minnesota’s goal is to create opportunities for Minnesotans to move from institutions to their own homes in the community. Moving Home Minnesota promotes the development and implementation of transition plans that reflect the preferences of those receiving services and the opportunity to receive services in the most integrated setting. We provide Transitional Coordination and Comprehensive Community Support Services (CCSS):

Eligibility

To be eligible for MHM services:

  • An individual must have resided in a qualified setting or institution for 90 or more days.
  • An Individual must also be eligible for Medical Assistance (MA)
  • Chooses to move into the community
  • Chooses to receive services
  • And more

Qualified Setting or institution:
A qualified institution can be any of the following:

  • Hospital
  • Nursing facility
  • Intermediate care facility for people with developmental disabilities (ICF/DD)
  • Institution for mental disease (IMD) such as Anoka Metro Regional Treatment Center
  • Child and Adolescent Behavioral Health Services facility in Willmar (CABHS)
  • Community behavioral health hospitals
  • And more
